Coast Shelter’s Client Referral Form is to be completed when requesting a specialist homelessness or domestic and family violence support service.
This Client Referral Form enables the client or referring service to provide our team with general information only. To complete the referral process, we will be in contact to collect further information. If you are having difficulty completing this form, please phone 02 4326 5395 between 8am-4pm Monday to Friday for assistance.
